Becoming a Patient


  • Please contact our office by phone or through our contact form – someone from our team will get back with you in 3 business days to review fees,  cost of care, services and schedule your first appointment.


  • A non-refundable $150 deposit is required to hold your new patient appointment. Your deposit will be  applied to the cost of your new visit. Payment is accepted by check, cash or credit card. If your appointment is  cancelled for any reason  your non-refundable deposit can be applied to a new appointment provided it is rescheduled within 3 months of the original appointment date.


  • Once you establish as a patient you will be invited to our online patient portal which gives you access to our comprehensive online forms and questionnaires, visit summaries, handouts, documents and more.


  • 5 days prior to being seen by the doctor, you will need to complete all of our online forms and ensure we have received your appropriate medical records.


Required Intake Forms 

Establishing clear agreements and preparing for your visit is an important part of starting a healthy relationship.  As part of our service, we offer an online comprehensive intake form, questionnaires, care agreement, medical record release, medicare opt out forms through our patient portal.  There is nothing like hearing about you from you, in person. This will happen during your visit with your physician. However, when we have  familiarity with you before your visit then we can make the most of our time together. Telling your story and providing a good written history is required on all new patients.

Once you make your appointment and have made your deposit, we will send you a link to our online patient portal. Our forms are depth and specially designed for functional integrative medicine.  Filling out the form can take up to 2 hours so please plan accordingly.  Do your best to fill it out as thoroughly and honestly as you can. Please know that they are private and only read by your physician.

Included in your visit fee is the additional time your physician spends preparing before your visit as well as creating an after visit summary – all of this can take up to 3 additional hours.   You are not additionally charged for the physician’s preparation  time provided we receive your forms and records no later than 3 business days prior to your appointment.  If  we do not receive this information in a timely way, you run the risk of your visit being automatically cancelled and forfeiting your deposit.
